What is Hope Enacted and what do we do?

Hope Enacted is a wraparound facilitation agency based in Colorado Springs. We work alongside children and families who are enrolled in the CHRP waiver to help build a team around your child, coordinate their care, and connect you with the right services and support. Think of us as your family's guide and advocate through what can be an overwhelming system.


How do I know if my child qualifies for wraparound services?

Your child may be eligible if they are enrolled in Colorado's CHRP (Children's Habilitation Residential Program) waiver and have been assigned or are seeking a wraparound facilitator. If you're not sure whether your child is enrolled, reach out to your care coordinator or county case manager — or contact us and we'll help you figure it out together.


How do I get started with Hope Enacted?

The first step is reaching out. We'll schedule a free initial consultation to learn about your family's situation, answer your questions, and determine if we're a good fit. If we move forward together, we'll handle the intake paperwork and connect with your care coordinator to get things set up. You don't need to figure it out alone — that's exactly what we're here for.


What is the CHRP waiver?

HRP stands for Children's Habilitation Residential Program. It is a Medicaid waiver in Colorado designed to support children with significant behavioral health needs by funding services that help them stay safely at home and in their community rather than in a residential facility. Wraparound facilitation is one of the services the waiver pays for.

Who pays for wraparound facilitation?

For families enrolled in the CHRP waiver, wraparound facilitation is covered at no cost to you. It is billed directly through your child's Medicaid/waiver benefits. You will not receive a bill from Hope Enacted.


What is a care coordinator and how are they different from a facilitator?

Your care coordinator is assigned through your county or a Medicaid managed care organization — they oversee your child's waiver eligibility and the overall service plan. A wraparound facilitator like Hope Enacted works directly with your family day-to-day: attending team meetings, connecting you with resources, helping problem-solve, and making sure the services on your child's plan are actually working. We collaborate closely with your care coordinator but we're a different role.


What does a wraparound facilitator actually do?

We do a lot. We help build and facilitate your child's wraparound team — the group of people (family, providers, school staff, community members) who meet regularly around your child's needs. We attend and run those team meetings, help develop and update your child's wraparound plan, connect your family with community resources, advocate for your child when barriers come up, and check in regularly to make sure things are moving in the right direction. We're in your corner.


What does the process look like from beginning to end?

Wraparound facilitation moves through phases. We start by getting to know your family — your child's strengths, your goals, and what hasn't worked in the past. From there we help you build a team around your child and create a strengths-based plan together. Over time, as things improve and your family builds confidence and natural supports, the intensity of facilitation naturally decreases. The goal is always that your family ends up more connected, more capable, and less reliant on a facilitator — not dependent on us forever.
